Ahoy. Welcome aboard the SS DoomRL.
Anyway, couple of pointers before I go back to my cabin...
You're carrying far too many medpacks. Once you have more than, say, 6, you should stop hoarding them and just use them as you come across them.
You'll probably want some power cells unless you're really serious about this whole "Shotgun and RL only" thing. The BFG just makes some late-game stuff so much easier as long as you have the cells to feed it with.
Start going in to more special levels, most of them are worth it. The only ones I can see newbs having trouble with is The Wall, Vaults and Mortuary.
I'm not sure if you're already doing this, but make sure you hit "Tab" during tough fights. Running mode makes you miss more often (Not a problem in shotgun games) but also dodge more oftenly too.

medpacks: About 3 large meds will do.
you don't need R1 on your Doubly Shotty if you have reloader AND HR AND Shottyman
Special levels = more exp, but flee Hell's Arena early if you think it's getting too hot.
The speed mod on the RL could have been put on the shotgun..
other than that, beware that you run outta ammo and have to run past stuff/melee them.

Yeah, I knew the reload pack was a bad idea, I don't actually remember why I did it, lol. Most of the mods I were slaping on was jsut for early experimentations with modding.
I'm pretty deadset on running with Shottyman and Double Shotty, as thats gotten me the farthest so far. the RL was just a nifty find I put to replace my incredibly neglected pistol. I would've loved to take the first plasma rifle I found, but it was already out of ammo when I killed the commando, and there were no energy cells in sight. :(
Ok, I figured I'd carry the medkits for "jsut in case situations", but I usally just found myself scavenging for small health globes at like 60% anyway, so it was never necessary, I found.
As for special arenas, the only ones I found were Hell's Arena (which I avoided, since I had to fight cacodemons on my first run ever and was like ASDF), the Chained Court, and The Wall (which I also avoided since I was doing so good and didn't want to risk dying)
Still, thanks for the tips, I'll try to improve my game. :D

Hell's Arena: Yes, scary w/o a chaingun and approprite traits, but i guess you can make do with some armor, medpacks and a combat shotgun.
Chained Court: uh....easy/free exp? It's just humans and sergeants. Dunno about the real high levels though, so correct me if i'm wrong. Yes there are some cacos/barons, but they kinda caged.
The Wall:Now, if you don't know what you're doing, you will 1.Die, 2.Destroy the shiny Backpack!. So, what you do is, fire at the bottom or top of the Wall, make a small gap then proceed to wipe out those Barons/Viles/Hell Knights. I don't know how to do "code" pictures, so....
