Are those O's cacodemons?  That's a nasty vault - V's, M's, and O's - and on Medium!  Is there any conditions to spawning these (new?) nasty vaults?  I say if one like this spawns they better have a unique inside.

Your equipment/inventory looks ready for Cyberdemon, why risk getting the unique?  In any case isn't it much safer to use a barrel to peel away a corner of the vault without harming the contents within?
