Right now, you're in the peak of health! Unfortunately, fighting your way through hell is making you feel tired, weary and old.
You begin by selecting 15 perks. Every two floors, you lose one (while still respecting prerequisites). Every three floors, you either lose an inventory slot, dropping whatever you were holding in that slot onto the ground.

Good grief! I'm assuming you are meaning 12 months of real-time?

And Doomguy needs to eat at least every 8 hours, otherwise he dies and you have to start it ALL over again.
