Here's my theory on how to get Dragonslayer? *SPOILER*
Spoiler (click to show/hide)I saw that people got it after failed attempt at Angel of Death on AoB and it spawned couple levels later - so requirement would be AoB + escaping UC? Kinda stupid, but anyway it's logical if based on those examples (just a though, I didn't dig into it really).

Most people get the thing on Ao100 so that can't be it...
