I'm a little on the fence about whether this is a problem or not but I've lost a number of characters within the first two enemy attacks because there was no escape from a caco vault or demon vault that I started in around the first 5 levels or so. Does anyone else have an opinion on this? Is it worth debating?

It sucks - but that's RNG goodness at it's best. Not an issue to my opinion
This happens to me on N! most often

There's a good chance this'll faze out because of the possibility of spawning in a locked vault (see here). It might be possible to make it specific ONLY to locked vaults, but it's probably for the best if you just don't spawn inside of a vault. Not that spawning in a vault is the WORST thing that's happened to me...

Stairs spawning you right into a vault are neither Roguelike nor Doomlike.

What are you talking about, the RNG throwing you into an unnecessarily difficult situation is entirely roguelike. :D

In Nethack, the 'vaults', while small, special-purpose and hard to access, never had a stairway inside them. In Moria/Angband and all the variants I've tried, none of the vaults I've encountered have had stairs leading in or out of them without magic or scroll trickery, and I'm willing to bet none of them anyone has ever conceived has. I don't think actual Rogue had any vaults at all, but if there were I think they'd follow similar rules.

In Doom, you are rarely thrown into instant combat upon entering a new level without any kind of place to cover and catch a breather nearby.

That is my logic behind my previous post; that is 'what I am talking about'. The fact remains that whether or not it's roguelike or doomlike, it may or may not be DoomRL-like. And it's just my opinion.
