What happened to the former lieutenants?

This is one thing that always bothered me in Doom.

Did the former lieutenants get promoted to former captains? Did they all become former former humans? (revenants or the unliving dead) It's unlikely that the Earth's militaries got rid of them seeing as how the armies are the most conservative entities in the world.

This is all kind of sad. I feel like I should be the one who promotes formers to former formers.

Simple: just like real lieutenants, they're indistinguishable from PFCs fresh out of boot camp ;-)
