[0.9.9.4] Access Error

Crashed when I tried to fully reload my combat shotgun.  Attached is the error.log

Using Win7 Pro 64-bit with sound.

This is apparently the same error previously reported here. Please move any further discussion to that thread (e.g. if the circumstances described there couldn't have caused your crash).

In general, it is nice if you quickly check the known bugs thread or the recent bug topics to avoid duplicate posts. Of course, when in doubt, post a new thread :)

(Also, the version is 0.9.9.4, not 0.9.4.4)
