The Butcher's Cleaver was a great early find.  If I had just been a little braver early on, and if I had skipped the Chained Court (to avoid not killing the cacodemons hidden away) I could have easily had 100% kills.  Alas, it doesn't really matter because none of the medals I earned, uniques and exotics I picked up, or badges that I should have earned but didn't even show in the mortem, or even a record of the win itself, were applied to my statistics.

Edit: I suppose I'll go ahead and describe this run a bit more.  It was fairly easy in the early game with the Butcher's Cleaver, but imps and sergeants could still deal some good damage against me if I wasn't careful.  I think there was only one level besides the Chained Court where I left without killing everything, and that was really just because I started right next to the stairs and didn't want to take many chances.  Going to the Chained Court was really rather pointless with the Butcher's Cleaver; I was sort of thinking about going for the Double Chainsaw, but I realized afterwards that would be pointless since I would get the Longinus Spear anyways.  I suppose I could have berserked and walked across the lava to get to the bacodemons, but since I already left a level with the enemies alive, there wasn't much point.  Hell's Armory was simple, though an imp managed to blow up the door to one of the arachnotrons, causing a bit of a panic.  Then I simply released the ahambler, and waited in a doorway until he got close enough to smack, which happened surprisingly quickly and without any real issue.  The Hellgate was actually pretty easy thanks to the Butcher's Cleaver allowing me to take care of the demons and lost souls quickly without being bombarded by the cacodemons too much.  Then I simply took on the bruiser brothers, which was also surprisingly easy considering the trouble I undoubtedly have on my non-melee runs.

The Necroarmor was a great find, and I really didn't use much else after that unless I needed fire resistance or if it was too banged up.  Unholy Cathedral was simple.  For the City of Skulls I used my Butcher's Cleaver to easily clear it.  Visiting it was really a no brainer because of the cleaver and MVm.  The complex of mancubi, and really all mancubi, weren't an issue, which was pretty surprising to me (maybe it's just because they were too slow and didn't get to attack me much?).  The revenants were the real threats to me, and of course any arch-viles (those damn things always run away while spamming their attacks).  Since I had 2 fire skulls, 1 blood skull, and 1 hatred skull, I figured the mortuary would be easy. 

And it sort of was.  I used the hatred skull first, a little ways away from the center.  On the left side, I used one of my blood skulls to take out an arch-vile and a couple of the enemies he revived.  Then further left, there was a second vile (or maybe the same one that just got blown back a large distance; it was all pretty confusing, but I think it was a different one) who had a bit of an entourage.  I think this is when I used my second fire skull, which I don't think quite killed him.  Then by the time I got to the left side of the screen, there was a whole army of enemies revived and waiting to kill me.  I charged in with no regard for my safety.  I still never used the spear's secondary fire, though I was surrounded by enemies quite a bit and considered it. How does the fatigue effect even work (or does it even?) while you're berserked?  Anyways, I started getting my ass kicked (though I had plenty of health packs), but if I remember right I managed to take out one of the viles before I had to phase away (thankfully it took me to the cleared up left side, where I grabbed a supercharge before promptly running back into battle.  At one point, maybe before or after my retreat, I used my blood skull for some corpse clearing and a good healing.  I still had a lot of fighting to do though.  One would think friendly fire amongst the enemies would be a good thing, but it was actually pretty inconvenient because enemies kept getting blown away from me, especially when the arch-viles used their attack right when I got in melee range of an enemy.  Nevertheless, after I managed to take out the last vile, it was just a matter of some cleanup.  I grabbed the Angelic Armor, went ahead and took the nuclear BFG just in case (I found my nuke a long time ago, but a second one never hurts, and I had the inventory space), fully healed with a super charge, let my Necroarmor fully heal, used another supercharge, and marched on.

Since I actually planned for the Lava Pits, and since I don't have to worry about ammo taking up inventory space, I already had two envirosuit packs, plus I found another one close to the stairs which I used before entering.  Without any lava-walking drama, the Lava Pits were really easy.  The Lava Elemental didn't teleport away at all once I found him, so he died quickly.  And thus I was all set for a full win.  However, come level 24, a bunch of arch-viles and revenants tried to make it so that didn't happen.  I nearly lost my Angelic Armor and had to use a bit of healing, but I prevailed eventually.  My Angelic Armor was damaged enough that I didn't bother with it and just stuck with the Necroarmor, which was faster anyways.  I enter the Phobos Arena, go to activate my nuke, and almost use my lava element instead.  Thankfully I stop myself and do things in the right order, then proceed to the Hell Fortress for the final showdown.  I ran to the right, did a bit of zigzagging without much use, and started smacking JC with my Longinus Spear.  He fell quickly, and I celebrated my victory.  My copy of DoomRL was apparently not pleased with my success, so it didn't count my achievements.

Unfortunately after you run a mod you need to quit and reload doomrl to get anything to update on your character. I found out the hard way after a HMP darkness win...

How does the fatigue effect even work (or does it even?) while you're berserked?

If you become fatigued by using the Spear (or the Staves, for that matter) you'll become 'tired' but the berserk will continue as normal.  When the berserk finishes you'll be 'tired' still.  I vaguely recall that the berserker trait triggering while running does something similar.

When Berserker triggers, you become "tired" right there.
I'd say that if one has over 40% of running time left when it triggers, one should jump to "Cautious" instead.

Because it's annoying to turn on run, only to find out you got berserk from something you smacked down just right after. (And thus you're "tired".)
