The enemy that was left behind could've been a flying enemy/archvile in level 4 (i just waited until the entire level was flooded. i didn't hear anything though after waiting a couple hundred turns), but i'm thinking it was on level 2 in some remote corner. there was acid and lava everywhere and i just couldn't see any corpses. it probably resurrected to a small island somewhere in there right as i was about to leave.

Hmm...  You're giving me ideas...  Maybe, for my next game, I should try to play a standard Nightmare game with 100% kills!  :D
