You have the "Boom!" message displayed only if you see the wall which got hit with the missed shot. Also notice that there is "The missile hits the arachnotron" message, which means no, at least one shot missed for sure. Still, the arachnotron at 6 o'clock surely hit all its missiles.

Once I get over the 'hump' of the beginning of the level and all the enemies spread out, I'm perfectly fine (usually). But I'd like to learn how to deal with these sketchy starts a little bit better. So maybe it'd be cool if you guys posted a picture of your rough starts and describe how you prevailed (or died) if you like.

Aha!  But that shot did hit!  It just didn't hit me!  :D

(Okay, fine, point taken; it still didn't save me from being killed instantly before I got a move)  :)

This is actually a big difference between UV and lower difficulty levels.

On HNTR/HMP, if you spawn into a place you can't survive, you can usually count on the adjacent rooms being relatively safe. On UV, you can't; you have to fight in the room you're in, and running for a door is only going to get you in more trouble.

In the situation you described, I think your best option would have been to play dodgeball with the Hell Knight in the room you spawned in. Not a great option, but better than running for the doorway.
