Unfortunately, this is a bug with DoomRL's module system, not Inferno. It happens whenever returning to the main menu after playing a module (console mode only).
(One option to avoid it is setting MenuReturn to false in config.lua, but its not much better.)

This is a feature. I do have some plans to make it more clear what is going on in the next version.

oh ok I trought it was a bug because imp or demon never jumped on my face from nowere anyway great module got raped twice by demon pack teleporting in from of me. it is very hard but fun also

With this run's luck, I'm sorry I picked ITYTD, but the mod is just damn hard on everything else and I wanted to get through it for once. Of course, part of it was less luck than knuckling down and using the Staff of Souls... it made Acheron nearly damageless for a change.
I'm surprised I got so many medals. Technomancer's Heart was thanks to an invulnerability, Lightning Cross a combination of invul and a Stupid Marine Trick.
Ptolomea got really dicey... on ITYTD. It might have gone better if I had picked up the BFG, but by that point I had a burst laser rifle, so...
I kind of wish there were more secrets... I went over every inch of every special level looking for them and only found one I didn't know about.
Hell's Egress was a cool level, but with 2d10+4x9 at +6 accuracy and Fin(3), I got hit once before drilling the Simulacrum into a wall. If I hadn't promptly forgotten about the lava element, it could easily have been damageless, too.
Antenora went okay with a double shotgun, a shell box, and a hatred skull popped before commandos joined the party.
There was a really interesting level somewhere in the middle - like a hell-froze-over level, except the rooms weren't connected, so I wound up making passages with my laser rifle.
I probably should have spaded what hellfire packs do to armor.
