Some DS thoughts.
Spoiler (click to show/hide)I recall someone saying that DS appears in UC, but only on N! and with 100% kills. But that's a guaranteed DS. Maybe you just have to have 100% kills when you pick up DS? And, maybe, at least HNTR.
And you can wear BA only with DS in your hands.
Only guesses... I'm not a very skilled player, I can't handle N! so I can't even verify N! UC YAAM.

Spoiler: Clarification (click to show/hide)I got the guaranteed DS to generate once, so I know you don't have to have 100% kills. I think it's based on how much damage you've taken, since I was trying to get Untouchable Cross that game. (Win, taking less than 50 damage) I'm pretty sure it has to be Nightmare! difficulty for it to generate there, and I think you're right about only being able to wear BA if you have DS.
Equality:
as far as I catch from mortems, Spoiler (click to show/hide)not taken damage or kill rate limited guaranteed DS but visiting (and may be completing) some of special levels. But my skills are not enough for descending to N! UC - usually they kill me at level 2 or 3...

i still don't know if you have condition to make DS appear (in that case i think i discovered few of them because i made "particular" things that i remember in this run before found it) or you can find it just by luck.
For condition to yield it when i read you i said myself maybe i was lucky and have done something without know :p
