DoomRL Fatal Error FMODInit Failed - Error initializing output device

Crashes at start... Throws this fatal error up:

DoomRL Fatal Error
Reason: FMODInit Failed - Error initializing output device

log file attached.

any ideas?

I have this happen to me on an irregular basis.  But DoomRL's always worked the second time I tried running it.

Intermittent problems with fmod probably cannot be fixed :(
