First, here is my player.txt:
--------------------------- DOOMRL Player Info ------------------------------

  All kills       : 3539       Standard wins : 0       imp             384
  Melee kills     : 393        Full wins     : 0       demon           579

-- kill a total 40 of cacodemons in melee

  Difficulty level achievements                  Deaths    Kills
I'm too young to die! : none                   none      none
Hey, Not Too Rough    : reached level 15       40        1828

  Nightmare!            : none                   none      none
 -----------------------------------------------------------------------------

As you can see, I reached level 15, but when I select "Show Player" from the menu, "reach level 15" isn't grayed out while two other achievements are.  The problem is - I reached level 15 on Angel of Marksmanship challenge, but so far couldn't do the same in regular game (I don't get enough uninterrupted time at home lately, so my play is sloppy).  I will overcome this, but it seems peculiar to say the least.  Is it a bug?  I don't know.  Does it "not quite meet player's expectations"?  Absolutely!

FWIW,
    Zarin.

I don't know whether this is intended or not, but on some conditions, doing something in a challenge game could actually be easier. The best example probably being completing Nightmare! on Angle of Haste. And another one being winning a game on AoB.

Even though I think Fingerzam is right, his point doesn't account for Zarin's problem. Reaching L15 in AoM should clear both requirements (reach L10 in AoM and reach L15). I don't know if this is actually a bug, though. Maybe the L15 requirement should read reach L15 in a normal game. I also don't know whether it's possible (or intended) to clear two requirements at once.

We already have/had a topic about how DoomRL counts your achievements.
Excluding challenge games from the "reach level X" requirements is good, see Blades whining about his N! win ;D
