Well, first off, I did end up being forced to scrape by levels 13-15. Still went on a murderkilldeath spree, and I got levels quickly enough. I also found about as many bulk mods as I did red armors (which explains my four bulk-modded red armors). Also, hot damn but do barons eat up armor. I had to switch out my bulk-modded red armor every map because of them.
Anyway, where'd I fuck everything up? Hellgate, of course. While I was kicking lost soul, cacodemon, and demon ass, I got a level up, and saw two choices - Dodgemaster and Blademaster. Now, they're both good choices, but at the time, I thought that getting a guaranteed dodge was a better idea than free actions.
OH BOY WAS I WRONG. I was being ripped to shreds because of that choice (lost souls are absolute bitches in AoB), and used up all my large medpacks. I just ran past the Bruiser Brothers.
Of course, I wasn't gonna get a break, no sir! Barons and archv viles and commandos for me! With maybe one or two health packs (I quickly used them), I had to make a mad dash for health.
Oh hi commando.

Wear Gothic Armor with Gothic Boots next to arch viles and mancubi. You'll need all the protection.
Despite what you may think, double shotguns are entirely effective at long ranges.
Shotgun guys need hugs. Take off your armor and give them one!
There's always cells around the corner. Let loose with your high power BFG9000 at that former human!
You should always wait to use medpacks - I mean, nothing's worse than being less than fully efficient, right? Also, you can get by with 12 health. Don't sweat it.
Out of shells for your combat shotty with many many demons in front of you? Do a full reload - you're going to need all the ammo.
Get Eagle Eye on AoMC, to get rid of that last 2% of possible missing.
Playing pacifist? Get SoaB, to show those demons that you're too much of a man to take down!
When 'zerking, make sure to switch to your fists - you don't get the damage boost otherwise, just like in Doom.
Don't worry, the Trigun's alt-fire won't hurt you (aside from the -5 MaxHP, of course).
Arch viles still have the warmup time for attacks that they did in Doom 2. Peeking out from cover, firing at them for two turns, and going back into cover works.
Not enough room to store some bullets you found? Drop a large medpack - you're never going to need it anyway.
You can fire and move at the same time, just like in Doom. Take advantage of this by circlestrafing the Cyberdemon.
You don't lose accuracy when running in Doom, and you can run forever. The same thing applies in DoomRL.
Don't worry, the projectile splashes you see are completely harmless, like in Doom. 'Cept for the Cyberdemon's rockets, you still want to avoid those.
Arachnotrons aren't a big deal, so long as you use weapons like the double shotgun. There's still a pain chance, and shotguns trigger it much more often.
Chainfire groups of arch viles. You really do need the extra firepower for them.
You can survive nukes with the insane amount of armor berserk packs give you.
Just because you're fighting Hell doesn't mean you should be un-gentlemanlike. Always give off warning shots before attacking something, and make sure they can see you.
Stay in the mid range of arachnotrons, and never get within their melee range. If you're right next to them, you won't be able to dodge their plasma, right?
Stand next to walls when fighting revenants so you can swerve to the left/right and have the missile hit the wall, not you. Again, there's no splash damage, so you don't need to worry about that.
Speaking of the revenant, the Revenant Launcher locks onto enemies. Fire it in any direction. Also, fire it right next to you for the largest cone of vision.
Phase devices aren't worth picking up. If you need to use them, then you're doing something wrong. Homing phase devices are even worse - their only use is for being a lazy bum.

Okay, I get back into DoomRL, at medium. My first four or so runs balls up around Hell's Arena or earlier (luck is a bitch). This one went pretty well, up until level 22 or so.
THEN I GET AN ARMY OF ARCH VILES COMING STRAIGHT TOWARDS ME WITH REVENANTS AND MANCUBI OH FUCK OH FUCK OH FUCK
Get to level 22 - nothing bad seems to happen. Go to the middle of the room - everything's blown up, there's two invulns, and there are two-three revenants. First thing I do - switch to missile launcher, grab an invuln, and get ready to kick ass.
Ah crap, an arch vile? I hate these guys D:
Wait, why are there more revenants coming, and... oh fuck OH FUCK GET THE OTHER INVULN
(at this point, I've seen the other three arch viles)
One of the revenant decided to rofl my invuln out of existence when I stepped on it. Retort - two missiles to the face, along with two for each of his buddies.
Of course, my invuln wears off, and I take cover while the other revenants and arch-viles decide to bumrush me. One BFG later, and a good bit of them are dead, and I missile the rest of them, albeit without any medpacks anymore. I find another arch vile, kill him, and watch the level title go blue. Find two large medpacks, be happy.
Next map seems tame. Cacodemons, imps, zombie platoons, whatever. Eat infiniplasma (and that is a word now). Things are good, until I get near the building with the arch-vile and fatso in the mortem. I hold them down with my missile launcher, then get a bit cocky and move up, wanting to peck off some others.
OF COURSE I HAD GOTHIC ARMOR ON. They turn me into canned soup - mancubus doing the souping, arch-vile the preparing and sealing. And I was doing so well, too. I even had a badass plasma rifle and missile launcher D:

Imp fireballs won't hurt imps, Baron and Hell Knight plasma won't hurt either, Arach plasma won't hurt Arachs, Cacos won't hurt cacos, and so forth. However, all former humans can hurt any other former humans, and the spider mastermind can hurt other spider masterminds. And all melee attacks hurt all monsters regardless of the source.Note I said 'projectiles'. Hitscans don't have such protection.
Edit: Well, the first sentence could have been misleading, but the second sentence mentions the projectiles, so it's fairly clear that's what I meant.

Well, according to ZDoom at any rate, monsters don't hurt other monsters of the same species at all. No special flags seem to be needed - it's the default behaviour for monster projectiles. Nor in Chocolate Doom do they seem to hurt each other.
Again, it's a decision on logic or Doomishness. I'd say logic, but hey - I'm just the guy who checks to see whether DooMRL is being consistent with (Z)Doom :P

Leave the lever square and the effect will 'turn off'. In-game, the belt pops out of the ceiling when you pull the lever, and when you leave, the belt snakes back into the ceiling. For plasma, assume a power cable.
