Saving on the staircase in the special levels causes the level to appear again. Here's the mortem:

On level 3 he battled through a maze of rooms.
He then stormed the Chained Court.
On level 4 he battled through a maze of rooms.
On level 5 he battled through a maze of rooms.
He then stormed the Chained Court.
On level 6 he found hellish caves.
...
On level 11 he battled through a maze of rooms.
He then ventured into the Halls of Carnage.
On level 12 he found hellish caves.
He then ventured into the Halls of Carnage.
On level 13 he battled through a maze of rooms.

Its a known bug. Its been said before already!

As jake already answered it's a known bug. It comes from the fact, that visited levels are not saved to the save file -- so if you save and load there's a chance that a special level will be generated again.
