Lieutenant or Captain seems about the right level to unlock it; definitely not lower than Sergeant or higher than Major though.

Ok, then.  50-150% more enemies than normal (100% monster count = normal game, 150% = AoD on M, 200% on H, and 250% on UV) depending on the difficulty, and you have to kill every single enemy on a level to go on to the next, no exceptions.  I'd make N! 300%, but I think with the respawning it'd be nigh-impossible to achieve a 100% kill count win as it is anyway (I bet Blade would love a challenge like this though ;P).

EDIT: Those numbers aren't set in stone, though... >_>

Basic concept: Enemies respawn like in N! but only a certain number of times depending on the level of difficulty.  However, you must have a 100% kill count on all ordinary levels or else you will not be able to go down the stairs ("You feel the presence of life drawing you up and away from the stairwell..").  Special levels will need to be handled differently; I'm thinking that a good policy might be that if you kill one enemy in an area like the UC or the Wall then you must go on to kill them all, or you can avoid them entirely by simply going to the stairs ("You quickly descend to the next level before the stench of life overwhelms you..").  Of course, for a perfect game you'll avoid them altogether, but some may want to visit different areas anyway.  It could also be that you have to choose between killing everything or avoiding everything on all the levels, but that doesn't seem to have quite the same bite as needing a 100% kill count even with only limited respawning to proceed. :P

Blade made it sound like you have to exp camp on the second level if you want to have a chance at surviving long enough to make it to the deeper levels/Cyberdemon on N! anyway, and since he's the first (and IIRC the only) person to beat N! I think this is unnecessary :P .  I could see it working maybe with a new challenge on the lower difficulty levels enabling respawning enemies (Angel of Death! :D), but N! is supposed to be insanely hard, so no.

Intuition 3, however, allows you to see where monsters are in a level (so you can shoot out into the darkness and kill something).

My most recent game was crazy.  After maybe level 3 or 4 I found at least one large medkit, supercharge, invincibility, or berserk powerup on every level, and usually two or more types.  I was able to kill everything and anything I met with ease thanks to being so hopped up on those things; making sure to try and get supercharge first then invincibility when possible or whatever I could do to make myself stronger longer.  I even found an invincibility globe about 2 or 3 squares away from the exit on one level!

Then, I reached level 15, and tragedy struck in the form of an Arch-Vile.  I'd never met one before, and so feeling like nothing could touch me started blasting away like I had on so many other monsters.  I mean, sure it's meant to be the toughest 'normal' enemy in the game, but I was finding so many powerups, and I had a ton of medkits!  Surely I could take him on.  I found myself at 10% and decided that I needed a break.  I used one of my phase devices and ended up on the other side of a building; perfect cover to use one of my large medkits and reload my babies.  So then, feeling like round two would go smoother, I went around the corner and there he was.  I was untouched for a couple rounds of shooting, and this encouraged me.  I was getting hurt by all the flames, but he was getting hurt just as much.  I was getting very close to death but wasn't about to stop shooting; I was going to kill that sucker, and if I had to nearly die a few times to get more bullets in him, then so be it.  Technically, I would have been fine with this logic for that turn if the arch-vile remained the sole enemy in sight.  But then, a human commando came in from the far left (probably to see what was going on with all the mini explosions) and upon seeing me decided that being roasted on an open fire wasn't enough, so he added his own unique ingredients to be sure that I'd make a good meal after death.

* - Most of my games don't go so well, so I don't bother making memorable names (though now that I think about it, isn't there an option to automatically choose a name?  I forgot how to do that/where to look for that. :/)
** - One hit less and I'd have had a chance to use a medkit.  I really should have been more cautious...
*** - For those monsters that simply won't go down in 2 clips.  Ideally, anyway...
**** - I did have some armor on originally, but being incautious I forgot to put a better suit on.  Could have saved my skin a bit longer but those flames hurt.

Fortunately, I did make it to level 15 in AoMr, so I have one of the three credits I need to reach the next skill level.

Unfortunately there were just too many imps wandering around for me to survive long on level 3, even though I'd gotten a record 5 (small) medkits saved up.  That human sergeant, ironically, was the first enemy I saw, and only noticed him standing behind me after I'd been blown in range by an imp and took a step away (bringing me even closer to the shotgun blast).

My first glimpse of how bad it was going to be came from seeing 6 humans, 1 sarge, and 1 lost soul when I opened the door.  Turns out there were actually about 3 or so hiding in that room.  I only had 2 medkits left (had to use the first two to survive the first level; nooby, I know), and it didn't take long even with dancing around the entrance to die.  So, what is the best trait to take when starting AoB?
