When we play DoomRL, we can generally avoid doing some obvious things and do others. Avoid acid/lava, shoot enemies, use med packs when you need to and so forth.
For the most part.
Sometimes, though, you do something so unbelievably stupid that you just facepalm at the mere thought of doing that. Note that the thing that you post must be something that you wuld not do normally and it happened due to sheer stupidity, NOT due to the RNG screwing you over. I made this thread so people can talk about those without having to make a Mortem thread.
I was playing AoP on HNTR. The last few levels went very well, had a number of large med packs, some decent armor (Nano-modded red, was hoping for Powered red), when I got to the Cyberdemon. With my Gatling Gun + Eagle Eye (Cateye build, for anyone interested), I was doing well. But I had to start running when I got low. I got blasted by rocket splash damage, but was still in yellow. I got into position, started shooting...and got hit by a rocket that killed me instantly. Should have paid better attemtion to my health. And I was at a good position to win the game as well.

I was playing Angel of Masochism. I don't remember the build I went with, but I always went for dodgemaster just to pass Cybie without trouble. In one of the runs I finally got to Tower of Babel and Cybie spawned only a few tiles from me. I activated running just to be sure I can reach the best position and start dodging. I think I had full tactical set by that time and hellrunner 2 of course, so I was just moving from side to side in front of cybie, waiting for him to shoot.
He blasted me right in the face, killing me instantly. Then I realized that I didn't have dodgemaster in this run yet (either I skipped a special level or there were too many caves on the way...
Just imagine the speedy marine dancing in front of the confused cyberdemon and then getting vaporized by his rocket.

I descended to floor 23 on HMP AoB with low health. I appeared almost surrounded by lava barrels, with two pain elementals easily in range of them.
I started hitting them, but the lost souls surrounded me, and I was on about 3% health when I remembered the alternate power of my weapon.
I thought I was using Azrael's Scythe, so I alt-reloaded.
I wasn't using Azrael's Scythe.

Once upon a time, I came home after work, turned on my laptop and loaded a promising AoMC UV run. Got past Cybie, was just pondering whether Mortuary could be survivable, when all of a sudden the laptop turned off.
My batteries were low and I forgot to put the laptop on AC ...

While fighting some Mancubi, I got knocked into a middle of a large lava pool, which was previously generated by a massive napalm barrel explosion.
I was wearing Antigrav steel boots, and I had Cerberus plasteel boots in my backpack.
Of course, since standing on lava with antigrav steel boots which totally offer no protection isn't fun, I quickly swapped them with my all-resistant Cerberus boots.
"Your backpack is full! Would you like to drop Antigrav Steel boots on the floor?"

Never happened to me in DoomRL before. Getting killed by firing a rocket at a wall point-blank? Many times.

Ok, this one's pretty bad, even for a thread like this. It's been quite a while, but I still feel like a massive moron for doing this.
Angel of 100, and I had gotten my grimy mitts on a BFG10k. I didn't use it all that much because of the non-intelligent splash damage and the unpredictable spread. At one point I nearly blew myself up by stupidly firing it into a narrow hallway. You can probably see where this is going.
Level 100. About half of it was clear, and I knew there was a pack of enemies out of LOS on the other side of a river. Because I'm lazy, when I'm trying to "shell" an area outside of LOS with AOE weapons, I move the aiming cursor just one tile in the direction I want to fire. I thought the BFG10k would be perfect for shelling this are I couldn't see.
Yes. I blew myself up on the final level of Ao100 with the goddamn BFG10k.

Ouch! Bet you probably don't want to see a BFG10k ever again. Sadly, I still need to find so I can get one step closer to Armorer Gold. I don't plan on using it myself.

Yeah, I've never ever seen the damn thing as well, though I love all kinds of explosive weapons. I've nearly made the same mistake as IronBeer a couple of times with the revenant's launcher, but I caught myself every time.

My stupid things - all my tries at least to complete AoMark. Unbelieveable, I completed AoB, AoSh and AoPc, but I cant even complete Deimos :(
Something stupid happens everytime. Not so long ago I was fragged by sergeant, carring a backpack full of medkits.
Clearing City of Skulls, but on next level I die. It's my curse...

HUH?
I never even knew about SHIFT+M.
...ah. Looks like that quickkey is only in the keybindings.lua file, not in the manual. And the same goes for Wiki as well, the regular Keybindings page does not mention it but the keybindings.lua page does.
No wonder I never noticed it.

o_O first thing I do every new release - comparing old and new config\keybindings
raekuul, almost all times I didnt miss proper key - I ever had no thoughts about it.
Just press f until monster die, wait, oh, SHI~, I dead?

sometimes I did hit "d" when pressing "f" to slam some monster with pistols, normally dropping an "f" item, and losing a second. Nice thing was when it was the only stack of 10mm left on me.
My stupid thing was trying to rocketjump with a wall right at my side. Thankfully it still blew surrounding souls apart for me to flee elsewhere.

Playing Ao100, "alarm rings" level feeling.
About floor...35, so it's all good in regards to difficulty.
Pretty pimped out in regards to gear, plenty of rockets, good set of traits...I forget which build though...
Decide to wait out the enemies, so I take cover. I can hear Viles, Mancubi, etc far away.
Sound does not get louder...
Wait about 100 clicks of the wait key, nothing.
I go out and investigate... "Hmm...there's a wall in the way, I guess I should blow it up with my rocket laun...OH S***, THE EVERYTHING!"
Everything hits me, I die in one turn...mother...

This reminds me - I enter a new level and "see a familiar object in front of me". Armed nuke.
Well, I've beat AoRA several times and these random nukes have a much longer timer than those in AoRA so I treat that as a standard level.
Cue me using the wait command and getting nuked instantly since I upped the max amount of turns it's supposed to wait significantly.

Ok, since we're talking about nukage now, I've got another fail to share.
Angel of Impatience, doing pretty well, I forgot what build/equipment I had. Working my way through... I think it was mid-Hell, and I run across a Thermonuclear Bomb sitting on the ground. My immediate thought was: "Heeeyyy. I could use that to clinch a partial victory." The fact that I was an Impatient marine didn't quite click until....
Me- "g".
DoomRL- "They always said patience was a virtue.... Nuclear detonation in 10 seconds!"
I couldn't stay mad. Honestly, that was just funny, even to me.

KABOOM , first rocket missed me, two napalm barel explosions not far away, second rocket, main menu...

I remembered another time today before acid and nepalm barrels were introduced.
I was hurting at the time, looking for a med kit or a lever to heal myself. I find a lever in this tiny room (3-4 high by 5 wide I think) with a pair of barrels. I shove the barrels as far away from the lever as I can and then pull the thing. "BOO-O-O-O-O-OM!!!!" Fried DoomGuy anyone?

Maybe less of a stupid and more of a sudden thing that happened, but ~120-130 runs ago, when I was still trying to beat AoMr on HMP...
I reached the Vaults with the Arena Master's Staff. I was in a near-perfect position to win with tons of ammo and a few large med packs. So, I...well, here's the last messages.
You picked up a large med-pack.
There is a lever here.
You pull the lever... Green acid covers the floor! Argh!!! Acid!
Argh!!! Acid!
Argh!!! Acid!
You use a large med-pack. You feel fully healed.
There are stairs leading to The Vaults here.

Yes, even though I had nanofiber red armor, I was brought from 100% to 0 in one turn. Long story short, I will only open the vaults on HMP from behind one of the walls that don't open. FYI, it was a Mancubus.

It seems that Mancubi are the new Archviles of the current version.

Because an unassisted old Arch-Vile was not capable of instantly killing a 100%HP player, nor did they obliterate all the cover in a target's vicinity.

Well, one of the things I do if I get to Vaults with the Staff is use it AT ONCE, to use a bit of time while those mancubi/viles/revenants/arachnotrons are still warming up.

Well, one of the things I do if I get to Vaults with the Staff is use it AT ONCE, to use a bit of time while those mancubi/viles/revenants/arachnotrons are still warming up.
What I do is clear the side vaults first, as they can be opened without the staff. The I use the staff to open just the the middle one. You have much less to worry about that way.

Entered the Vaults with a bunch of phase devices in inventory, decided to attempt Scavenger Platinum - and forgot I was playing AoMC.
Yes, I did happen to phase in. Yes, I did get promptly splattered all over the walls.

Maybe this is something everybody has already seen.
Playing normally, a common game, and BAM! Being hit so hard that you're almost dead. I was in 5% health. I was able to evade the monsters' attacks and, luckily, found an invulnerability orb close by. I grabbed it and then I remembered something...
I was playing Angel of Purity. Cue "FUUUUUUUUUUUUU-!" face when a monster killed me.

So, playing Angel if Impatience, I wanted to grab Lava Silver (Lava Pits on AoI) as well as Eagerness Silver. I was playing a Shottyhead build and had a Super Shotgun + Assault Shotgun (From Mortuary).
I make it to the level right before the Lava Pits. I score a Supercharge, but no Envirosuit or an invulnera sphere. After some experimentation with rocket jumping, I decide on Angelic Armor and P-modded Plasteel boots.
Long story short, I didn't make it. *sigh*.

Well, managed to one-up myself again. I failed to notice that the Jackhammer I had was not fully loaded (UV AoMC). Not realising it, I shot a group of 3 revenants and 2 arch-viles that appeared right after I entered the level. You can imagine how well that went...

Well, I've had a few things that just screwed me up.
The first was when I first visited the City of Skulls on Hey, Not Too Rough. I'm still new to playing, but I thought 'This is the furthest I've gotten so far, this shouldn't be too bad.' Well, when you get swarmed, I didn't know what to do. I first thought of the BFG 9000 I got at the Halls of Carnage but then assumed the splash damage would kill me. I then thought my Trigun would do the trick since on the wiki, it said I nuke the level and only lose 5 HP permanently. Well, what it neglected to tell me is that it was a nuke regardless of a scant 5 hp and killed me anyway. I then consulted the wiki again and learned that trick about the BFG, then held my face for a bit.
The second time was, again, the City of Skulls. Having learned since last time, I thought I could do it again but I realized the missile launcher isn't the best for up close fighting, neither is the Acid Launcher. Long story short, I did eventually beat that level with 1 % left, and got a Blood Skull. I thought it would make me berserk and help recoup the pain I endured of almost dying. I went down the stairs and I'm surrounded by acid, and there's an Arachnoid just sitting there. I know he spotted me, and I only have seconds to figure out what weapon to use...But like Han, he shot first...
I neglected to include my first encounter breaking open The Wall...Eeesh, at least I died quick.

Pain Elementals freak me out when playing Marksmanship. So I run into one in a small corridor right next to a barrel and decide it'd be a good idea to blow it up.
And miscalculated the explosive range on barrels, causing the lava barrel right next to myself to explode.
Sigh.
