So, I played this one for a Quartermaster Platinum. I decided to use Blademaster to cut down on the amount of time that I would have to spend fighting.

He managed to clear the Mortuary from evil!
On level 22 he entered the Lava Pits.
He managed to clear the Lava Pits completely!
Then at last he found Dis!
He defeated the Mastermind and found the TRUE EVIL!
Then finally in hell itself, he killed the final EVIL.

I wrote this small explanation (http://forum.chaosforge.org/index.php/topic,4454.msg36618.html#msg36618) on corpse removal a long while back, but many of those things still apply. The big one in places like Hell's Arena is corpse-stacking: usually this means stocking up on ridiculous amounts of shells or bullets beforehand to make sure you can deal with each wave appropriately. I wouldn't know how 2Dev does it so reliably, but if I had to guess, it involves shotguns and making sure you kill things near walls so you can mitigate knockback. (With Bru2/Ber and a knife, he can probably do some positioning that way as well.)
Still don't know how you manage to pump these out as efficiently and as quickly as you do: even with melee, a 96% kill rate seems absurdly high. I hope you liked the new ripper!

I both corpse-stack by leading demons over corpses and I also dodge back and forth to get Cacos to miss me and gib corpses, in addition to using shotguns to knock enemies back. By now I can predict with a good deal of accuracy where an enemy's corpse will go when I kill it with a shotgun, and that really helps. In addition, I like to weaken enemies with the shotgun, and then deal the final blow with melee so the corpse drops where I want it to. It takes quite a bit of practice, but it's worth it when you master it.
As for the new Ripper, I really like it because then I can increase my damage output by doubling my attack rate. By the time I'm using it, I have enough Brute levels to cancel out it's to-hit minus and still maintain a 98% hit rate. Compared to the Longinus Spear, I got 14d6+30 vs the 8d8+15 that the LS does. The damage increase only becomes more dramatic when you factor in zerking, then it is (14d6+30)x2 for the Ripper, and (8d8+15)x2 for the LS.
For my ridiculously high kill rate, remember that by the mid/late part of the game I had a really fast move speed and I could one-shot everything around me each turn.

so, in order to wipe the Arena one needs to lead demons and cacos to either shoot at previous corpses to gib them, or to die over others' corpses so to leave no corpse? Owww, this is going to be hard to pull with my current skill. I wish there would be a door at the arena :) Ohh, one can use stairs as one! Hmmmm...

If you take a level in Hellrunner, it's not that hard, really. A scout with HR 1 moves as fast as the Demons, so once you're able to outwalk them, it's just a matter of luring them into the right position before dealing the death blow.
