Inspired by Mrazerty's trash medals (http://forum.chaosforge.org/index.php/topic,3434.msg33395.html#msg33395), I think this needs a topic of its own. These hypothetical 'awards' wouldn't be counted or displayed in your player info, but would result in some kind of silly/snarky one-liner being added to your mortem. Some marks would be excluded from certain challenge modes. (Sloth from Ao100, Waste from AoB etc.)
Mark of Cowardice - Quit the game on Phobos Surface
Mark of Soreness - Quit with 1 HP.
Mark of Temptation - Died from flooding the level with acid/lava.
Mark of Idiocy - Died by pushing a barrel into acid/lava.
Mark of Clumsiness - Died on a floor that was cleared.
Mark of Arrogance - Died within 10 turns of entering an optional level.
Mark of Sloth - Died after 8 hours playtime.
Mark of Regression - Died from a former human while clvl 7 or higher.
Mark of Misfortune - Died the same turn as using as phase device.
Mark of Waste - Died without any ammo.
Mark of Avarice - Died while holding 3 or more large med-packs.
Mark of Forgetfulness - Died nude and holding armor, both for over 100 turns.
Mark of Regret - Died standing on an invulnerability globe.
Mark of Reluctance - Died standing on the stairs.
Mark of Weakness - Died with a BFG in hand.
Mark of Recklessness - Died by a rocket jump.
Mark of Impulsiveness - Died by a nuke before level 25.
Mark of Ignorance - Died by firing a Revenant's Launcher beside you.
Mark of Decay - Died with a max HP of less than 10.
Add your own, provide one-liners, or admit to how many you've 'earned'!

Idiocy doesn't work as of latest stable (can't push barrels into liquid, period). That said...

I think this idea can add value to the game without beng just plain silly. I don't have much to add right now, but it's something I'm definitely thinking about

I agree. It's just the game taunting you. The game already does that, for example when you spawn right in the middle of a vault when your Health is below 10%.

This could be integrated with the ignominious history provided at the end of each mortem.
e.g. instead of '50 were killed':
13 were killed by hellspawn.
1 was killed by something unknown.
7 had luck that proved fatal. (lava lever/phase instadeath/killed from 100%/dead on stairs or invul/zerk)
6 didn't make good use of their resources. (dying with tons of medpacks/forgetting about armor/on a damage floor with an envirosuit, or without wearing boots in their inventory beforehand/with a fully-loaded unique or BFG in their inventory/maybe running dry of ammo in non-MAc)
7 didn't read the thermonuclear bomb manual. (mark of impulsiveness)
9 cocksure bastards got in over their heads. (maybe running out of ammo/dying within x turns of a special level)
4 couldn't figure out when to run. (100% kills until death level, killed by last enemy on level which was M/V/R/died with a homing phase in a dangerous demon cave or similarly bad place)
...
And 3 couldn't handle the stress and committed a stupid suicide. (mark of clumsiness, recklessness, idiocy)
Of course there should be some sort of one-liner in the 'current' mortem to tell you which statistic this YASD boosted.
I've earned remarkably few, myself, though I only have a few dozen mortems.

This is a good place for them - yes
However, I think this entire section probably needs to be reworked as it is beginning to read like a technical manual, probably because it's a holdover from the earlies DoomRL versions with things being tacked on every release.

Most of those I've never had, but I haver to admit to earning Avarice on just about every non AoI death - I love my medpaks.
