Title: Im sure this has been mentioned thousands of times But what about a nuke lever?

A lever that nukes the level.maybe its message to notify you of its presence could be something like "you hear the ticking of geiger counters"

Hmm quite honestly, that indeed doesn't sound bad, at least if it's rare. More rare than a thermie.
Anyone think it would cause any balance issues of some sort?

Balance issues ? Perhaps not at all.

The thing to bother with is if introducing another reasons of violent insta-death makes any kind of sense.
For what ? To piss players off completely ? Are reV-packs insufficent ?

What about floods level with *insert dangerous liquid here* levers?wouldn't they just piss people off?If it is put in though it should have a 3 turn delay so you can shoot that caco,use the homing phase device and get the fu** out of there.

What if you don't have a homing phase device? With "flood all with lava/acid" you at least have SOME chance you'll make it to the stairs using whatever medkits you have stacked. With a nuke lever it's a coinflip - either you have homing phase, or exit/invincability close enough to get to it before it blows, or else you die. No save.

I personaly would like it if it were choice dependant, but not just "woops, nuke time!"

I like the idea! There would have to be a message warning you when you entered the level, of course. And 3 turns is too quick. How about the nuke goes off in 1-3 minutes, randomly? That would give you enough time to run away in almost any level. If the staircase is too far, or you haven't found it yet, it's your own stupid fault for going around pulling levers when the level warned you there was a nuke out there.

Not really.  Considering that AoRA uses 5 minutes of game time as the standard, 1 to 3 minutes is rather short in comparison.  This will be even more pertinent when you consider that you need time to fire or dodge demons that are impeding your path to the stairs before the nuke goes off.
