Clearing Hellgate on UV while not taking damage during the level doesn't give the badge. What do you need to do to acieve it?

Well it is a diamond badge, so it's probably... get all the way to hellgate and beat it without taking damage the entire time.
I suggest a scout with many shotgun shells and perfect knowledge of corner shooting. Doing it on max carnage probably make it easier.

There are numerous cases of getting Gatekeeper Diamond: I have received it on a couple of occasions myself, and I can say that all you need to do is kill everything at Hellgate, then leave, without taking damage (and on at least the Ultra-violence difficulty). If there are ANY damage sources at all, whether direct attacks or splash damage or self-inflicted hits, this will prevent you from getting the badge. If, for some reason, this is definitely the case and yet you still didn't get the badge, then that would be a bug and you should report it.
I haven't heard of any cases where this is a bug, so it's possible that you took damage somewhere without realizing it.

Also, try not to enter Hellgate above 100% health without Badass because the natural health drain could be considered 'damage' for GK Diamond purposes

Reporting in Bug Reports now- Happened AGAIN. Had Badass, grabbed the health globes at the start (Because i can) And maintained 117% HP the whole way through. No GK Diamond. (Yes, I used the zap-through-walls-while-hidden-with-plasma-rifle-of-ownage tactic.) Do i possibly have to win the game too? (Both times i got my ass kicked in phobos hell)
