What does this mean (you can find it in the "Statistics" part of a Mortem)?
ToHit +# ToDmg Ranged +# ToDmg Melee +#
... and why is it in most cases respectively 0,0,0 or 4,3,3?

ToHit - bonus to chance to hit, affected by EagleEye. People most usually pick EE*2, each level of EE grants 2 points ToHit so it's 2*2=4.
ToDmg Ranged - bonus to damage dealt by ranged weapons. Affected by SoaB. 3 levels of SoaB usually, each grants +1 damage, that's why it's 3.
ToDmg Melee - same, but for melee. Affected by SoaB and Brute. Because people rarely pick Brute in non-melee games, it's 3, as ToDmg Ranged.

I'm afraid SoaB does not really affect melee damage.
He killed 683 out of 695 hellspawn. (98%)
He held his right to remain violent.
He was an Angel of Berserk!

If I'm not mistaken, they actually reduced Brute down to +3 instead of +4 and just forgot to put it in the trait's listing.
So yeah, SoaB does still affect melee damage.
