Title: [0.9.9.1] Enemy telefrag = Me Out of Place?

My guess as to this message is that a monster teleported (note I haven't been anywhere near the only teleporter on the map) and died due to an incompatible teleport (or killed another monster in the process).  The fact that "You feel out of place" appears is probably bug-related.  After playing through the level, I was able to determine the teleporter output tile, which was not on the same tile as the one I'm on in the picture.

This seems a bit odd, and to be honest, I have no idea what the hell the bug could be if it is one.

I'll get Kornel's input on it, and track if necessary.
