I've read about spawns in locked vaults being fixed, but don't know if teleporters are accounted for, and I've just encountered this today for the first time.

Probably, but that'd make teleporting like even more stupid.
Each time I find a teleporter or non-homing phase device I'm like "Oh handy! I was just needing something to wipe my butt with!". So at least we could try making it more useful rather than pointless or even risky.

You're exaggerating the problem, though. As it is now, phase devices are pretty handy for quick getaways, while locked vaults aren't exactly as common as them. The whole locked vault problem doesn't make them toilet paper; it's more akin to lighting a match in an area with a slight fire hazard.

Phase devices will never put you into a locked vault.
As the OP found, teleports can, but this behavior seems unintended. I'll create a ticket.

I dunno man. I'd still rather pick up a small medpack than a phase device. Maybe it's just my luck, but phase devices ALWAYS seem to get me even deeper into shit.

Perhaps the simple solution would be to allow a way to get out of a locked vault if you manage to get in prematurely? Maybe the walls can just be opened from the inside, or there's a twin lever that disappears with the one outside the vault.
But yes, I agree that the game should never put you in a situation that makes the game unwinnable and forces you to quit.
