I am playing v.0.9.8.5 on Windows XP and sometimes the game crashes on the first level. I haven't been able to figure out what might be triggering it, I'm just shooting the things as usual. Once I get by the first level it hasn't happened.
Here is what it looked like last time:

I was standing about where the error message starts printing, right uner the 'A'. I suspect that
An unhandled exception occurred at $77BD2237 :
EAccessViolation : Access violation
is the important part.
This problem is happening at a frequency that is alarmingly close to 50%.

Woah, that's a very alarming bug :-|. When did you download the exe?

Unfortunately, I did not download it recently. I love the game but I sometimes set it aside for more pressing issues. It has been a while since I updated to this version and I can't remember when it was, but the top three lines of the version file say:
v.0.9.8.5
(27.XI.2006)
(16 bugfixes, 11 modifications, 18 additions)
Which looks like this version was released on November 27, so it could not have been that long ago.

There was a crash bug that was present in the first 10 downloaded zips (I updated it immidately after release). So you may try to redownload the zip (you need only the exe). Also it would realy help if you'd note the linenumbers and filenames (there should be ones on the screen when you crash).

I only hesitated to download a new copy because these crashes are not reproducable on demand. I can't see what triggers them, so I will never really be able to know if they are gone. Perhaps 50% was a big high, but I did get two of them in the same day once. If I download another copy I might never be able to discover what causes this crash. Perhaps that is silly.

That's a bit funny! If you look to the top of this thread, you may notice that I gave you the screen I got when I crashed. I don't know how to interpret any linenumbers and filenames from that, but if there is a way then I expect you'll know it and should be able to do it yourself.
Perhaps this is a bug within a bug, a crash that avoids the usual crash-report mechanism.
