I was playing this game the other day and was awarded a Fallout Gold Cross even though I only nuked 2 levels.  It's done this before; I think it has something to do with the fact that I nuked a generated level instead of a regular level.  (In this caes, the Lava Pits)

I'm pretty sure this guy's on the money. Technically you don't have to survive three/six nukes, you need only have three/six somewhere on the map during the game (including Trigun nukes, although you'd have to survive those).
