I used a nuke on level 25 while invulnerable, then used a nuke on the final level. The game said "You die..." I can't remember if that's normal. Then it went straight to the mortem, but it was the mortem of my previous game from a few hours ago. My full win mortem file wasn't in the folder.

Is this a bug, or can you no longer nuke the final level? It should still produce a normal mortem of course.

Is there anything else I can post since there's no mortem file? I haven't started a new game yet.

Would the error log disappear if I restart the executable? Now that I think of it, I did restart the game once but didn't begin a new one.

Okay, hold on a second here. I just got another full win and after the first nuke went off, I had a sinking feeling in my stomach: I don't remember going down the stairs to the secret level in the bugged game in the OP. Could I have armed the second nuke on level 25?? Maybe I forgot about the stairs and thought it automatically teleported me in there. If I was standing on the left side I might have mistaken it for the final level.

I was using the lava element. It doesn't teleport to the next stage automatically or anything, right?

I'm not exactly sure what happened. Sorry for the confusion. But it should be easy to check whether 2 nukes on level 25 will fail to produce a mortem. If not then maybe I did find the stairs and the OP is the correct account. I guess I'm out of it today.

AFAIK, you still have to walk, but the botched mortem is weird nonetheless.

I just tried it, double-nuking 25 caused the glitch as described in the OP.  I wasn't credited for the new (to me) assembly I made in the run either.  Can't be sure about enemy kills / other items, but probably not.

Yeah, I think I was most disappointed about the missing medals and items I found. It'll take me another 75 hours to get my first Hellwave Pack again! Haha.

I feel dumb about ruining that game, but at least we figured it out.

I have not been able to reproduce this bug...I've gotten a different bug. I double nuked a few levels to see if it would happen on any level, not just level 25. I found that you could live through both explosions, the game had no problem, and would continue.

So far I've been unable to find any other level that will glitch if you die from a second nuclear explosion. But there is a glitch of some sort when you die from nuke #2. A mortem did generate for my character...or at least appeared to. I was actually presented with the mortem of a character I'd played around a month ago, who died in the arena. So...yeah. Bug of some sort. A ticket has been created, but please report more information on this bug so we can squash it.

EDIT: Sorry, misread the original post. The glitch is the same. I thought that the mortem report wasn't shown at all.
