Blade:
Keys could be nice in fixed precreated levels. As an example of key use: Hoc's BFG room could be smaller:

, so it's great chance to destroy BFG if you will try to open it with rocket(but if you're lucky or experienced, it is possible). But there is a room on the level, where are 2 or 3 BoHs that are guarding the key, which safely opens the door.
In future(when an ability to create custom levels would be implemented), keys could be a very nice way to create interesting levels.
BDR:
Ooh, suddenly a picture in my mind: A maze level filled with mini-vaults, and also lots of mancubi, who coincidentally are guarding the keys to all the mini-vaults.

Another idea for a challenge game: When you go down to another level, you lost almost your all inventory. For example, you don't lost your equipment items, two items of ammo for your weapons and the thermie.

Angel of Speed - Get exp for diving not for killing. The quicker you dive more exp you get.
Angel of Teleportation - Teleport randomly every so often.
Angel of Vulnerability - Not allowed to wear armour or boots
Potman:
Angel of Conflict
- Always in Aggressive mode
- Enemies are considered as they were in Aggressive mode as well
- Enemies sometimes attack each other when the player's around
- More enemies are generated
- Stronger enemies are generated at earlier levels
Angel of Vengeance
- All damage gains a +1 bonus
- Additional +1 bonus on each level up
- All enemies killed will respawn to the next level
Angel of Love
- Always a level up when descending to next level
- Enemies can't be killed or harmed, with the exception of CD and JC
