I think it's 1/3 of MaxHP before any resistances or armor are applied. TaN and the berserk status itself won't affect the injury-based procs, but Ironman will. Also, anytime you get hit and the message "That hurt!" pops up, it means that last hit had a chance at proccing Berserker.

I think Iron is correct - for example, mine was proccing on Cyberdemon rockets that did no damage whatsoever in a complex I found. I was Berserk for the next 7 floors.

However, those rockets would have easily done the requisite percentage of my HP as damage without Inquisitor.

Yup.  Also, the berserk resistance bonuses will apply to the damage that procs it.

There's no chance to proc -- it's guaranteed provided the attack has enough potential damage.

Thanks to this thread, I've finally figured out the secret. Everyone saying that it's dead simple is, well... not lying at all. It really isn't worth your while to overthink things (though I did really like the idea of dying on top of it and being resurrected with it in hand. Idea for a new super unique?)

That said, back when I was still pondering 'HOW?', I was thinking there might've been clues squirreled away not in Doom RL, but in a certain other project of Kornel's that's up at Chaosforge (aka the one based off where DS is taken from). There aren't any (as far as I know), but hey, if the dev's are feeling particularly evil when they're doing the next build...
